The Supervised
Group Living Program (SGL) at Southern Hills provides mentally ill individuals a
therapeutic environment in a homelike residential setting. Southern Hills has
four residential group homes providing 34 beds. Each of these SGL homes is co-ed
and handicap accessible. The program operates 24 hours a day, 365 days a year
with around the clock staff coverage.
Each resident participates in the program according to his/her individual needs.
The program is open to ambulatory adults, both male and female. Qualified
individuals will have a diagnosis of mental illness, a psychiatric disorder that
substantially affects a person's thinking, feeling, or behavior and impairs the
individual’s ability to function and may, as a result, have difficulties in the
areas of daily living skills and interpersonal relationships.
Each resident will participate in the SGL program according to his or her
individual needs. The overriding goals of the program are to: a) provide housing
and the necessary supports to sustain residents in the community; b) minimize
the consumer’s psychiatric symptoms; c) prevent psychiatric re-hospitalization;
and d) promote a satisfactory quality of life.
The services of other Southern Hills programs are made available to each
resident based on their individual needs. This includes Rehabilitative Day
Treatment, outpatient and group therapy, medication management, case management,
and education. All group home residents are assigned to a Center physician, a
primary therapist, and a case manager.
Public transportation is not readily available in our communities; therefore,
Southern Hills provides transportation for the group home residents through case
management services and Center-owned vans. These transportation opportunities
increase the accessibility for consumers to Southern Hills programs. Consumers
may also use these transportation opportunities to attend various community,
recreational, and religious functions.
